The Opportunity

Aveo Care at Home is an approved provider of Home Care Packages (HCP) to residents throughout our Aveo retirement living communities. Our goal is to deliver tailored support that helps residents remain independent in their own homes for as long as possible. As a Case Manager, you will be responsible for managing a caseload of home care packages, providing support and guidance to access services, and promoting Aveo as a home care provider. You will enjoy working closely with residents to develop personalised plans, coordinate services, and ensure residents receive tailored support to remain in their Aveo home. This is a rewarding opportunity for a Nurse with a background in Home or Community Care and knowledge of Home Care Packages who is looking for a resident-facing role.

This is a permanent full-time Monday-Friday opportunity based at Concierge Balwyn Retirement Living, located in Balwyn.

You will:
  • Meet with prospective Aveo residents and their families to understand and assess their care needs
  • Provide tailored information on care services and funding options available
  • Establish home care plans, ensuring supports are in place as required and complete regular scheduled visits
  • Prepare and maintain records, documents and reports as required 
  • Deliver excellent customer service to residents, families, staff, management, visitors and external health professionals
  • Liaise effectively with internal teams and external providers who provide care within the community
  • Promote Aveo Care at Home by attending community events, assisting with the transfer of home care packages and providing support throughout the My Aged Care journey

What you will bring:

  • Current AHPRA Div 1 registration with experience preferably in a Home or Community Care setting
  • Demonstrated experience in understanding care plans and clinical assessments
  • Solid understanding of My Aged Care and Home Care Packages or similar commonwealth funding package models (e.g CHSP) 
  • Excellent communication and technology skills. Experience with Alayacare is a bonus!
  • Current CPR Certificate and two (2) COVID Vaccinations (required)
