Aveo is seeking an experienced Care at Home Nurse to join our team at Aveo. You'll be passionate about delivering excellent service and meeting our consumers needs. 

About the Role:

Operating across Aveo communities throughout the Sydney Northern Beaches area, you will proactively deliver exceptional person-centred home care case management for our consumers within Aveo retirement communities and the wider community. This rewarding opportunity will enable you to actively achieve quality of life for our consumers and work with an experienced team.

Some of your responsibilities will include:

  • Meet with prospective consumers and their representatives to understand their care needs, eligibility and assist with referral and navigation of the My Aged Care system.
  • Provide case management for consumers who receive government funded Home Care packages with Aveo Care at Home
  • Assess, review and evaluate care and services provided to consumers
  • Prepare and maintain records, documents and reports as required
  • Ensure a consumer-directed approach to service delivery and quality care in accordance with Aged Care Quality Standards, Legislations and Aveo's care governance framework

To be successful in this role you will have:

  • Current AHPRA registration
  • Demonstrated recent experience as a Registered Nurse in a community nurse or case management role in the community sector
  • Clinical nursing experience within a community care setting
  • Home Care Package case management experience in a community setting desirable
  • Comprehensive knowledge of the Commonwealth Home Care regulatory framework, including funding and Aged Care Quality and Safety Commission requirements
  • Demonstrated problem-solving skills related to clinical service delivery in the home
  • High level communication, interpersonal and organisational skills
  • Ability to liaise proactively with key stakeholders to achieve required care outcomes
  • Demonstrated understanding of continuous quality improvement practices and application
  • Commitment to customer service and achieving positive consumer outcomes
  • Experience in networking and promotion of services
  • IT literacy with an ability to manage spreadsheets and use Microsoft Office applications
  • Current Police Check certificate, or willingness to complete one
  • Current CPR certificate

All current employees are expected to have received their first vaccination by 1 Nov  2021 and their follow up vaccination by no later than 15 Dec 2021.
